I think he would be classed as category 1 because he probably would have bled to death without Miracle Day.
I think he would have suffered a lot of injury in having his eye removed - the Gentleman even lamented that, because of the Miracle, he would be conscious the entire time, implying that the same injuries would have killed him if not for the Miracle. I think it's safe to say he was a Category 1. It's possible that, like Rex, he recovered enough to survive when the Miracle ended, but there's no definitive proof of that.
